DMCA Generator allows you to format your DMCA copyright infringement notices by inputting basic information in the relevant textboxes, submitting it, and then printing the resulting page. Follow these simple steps to use the DMCA Generator:
1) Enter your name in the "Name" field.
2) Enter your full mailing address in the "Postal Address" field, including street address, city, state or province, country, and postal or zip code. For example, 123 Fake Street, Albany, New York, United States of America, 55555.
3) Enter your telephone number in the "Phone Number" field.
4) Enter your Fax number in the "Fax Number" field or "N/A" if you do not have a fax number.
5) Enter your e-mail address in the "E-mail Address" field.
6) In the "Original content URLs" field, enter the precise URLs of all of the content you claim was infringed. Use a separate line for each URL. For example:

7) In the "Infringing content URLs" field, enter the precise URLs of all of the pages you claim infringe your copyrighted content. As in the "Original content URLs," use a separate line for each URL.
8) Click "Generate DMCA Notice!" to format the information you enter.
9) Print the form, sign it, and fax or mail it to the ISP, webmaster, or other relevant individual or entity who you are addressing this form to.
Some ISPs or websites accept e-mailed copyright infringement notices and allow the sender to type their name where they would have otherwise signed it. If the intended recipient allows you to e-mail them with a printed signature where you would otherwise sign with a pen, simply copy and paste the formatted page into your e-mail software, type your name where you would have signed, and send it to that recipient.
